Philosophy

Circle of Care is a network of provider agencies, taking the lead in constructing a continuum of care and services for HIV impacted families using a family-centered model based upon the following beliefs:

  • Provide comprehensive family-centered medical care and support services to HIV infected women, children, youth and families.
  • Provide services to reduce the risk of perinatal transmission for pregnant women and women of child-bearing age.
  • Provide culturally competent HIV counseling and rapid testing to impacted communities.
  • Ensure that HIV positive women, children youth and families know their HIV status and are linked to care and support services through coordinated and targeted case-finding.
  • Encourage and support consumers to experience a sense of community, support and development of consumer leadership, and empower consumers to advocate for themselves, their families and their communities.
  • Ensure quality of care through the provision of care program support services, planning, evaluation, and the coordination of services.
  •  Ensure quality outcomes for clinical care, case management and other services.

 What is family-centered care model?

The family and its well-being is the center from which Circle of Care constructs it network of medical and specialty care and supportive services. The model is designed to engage both the individuals with HIV and the natural support network of the family to be active participants in the care team. Every family served by the Circle has a care coordinator whose primary tasks are to:

  1. focus on identifying and supporting the consumer in removing barriers to care;
  2. assist in supporting adherence to medical regimes and medications; and
  3. to assist in coordination of and access to other benefits and programs as needed.
