Our Mission Circle of Care is committed to the provision of quality comprehensive family-centered care and services for HIV affected women, children, adolescents, and their families and to the prevention of HIV disease in these populations. History Circle of Care was founded in 1989 as a network of health and social service providers with an exclusive focus on women, children, youth and their families. In 1994, Circle of Care expanded it services to include perinatal HIV prevention in the care of HIV positive pregnant women. With continued assessment of need, Circle of Care recognized the need to provide HIV counseling and rapid testing in 2006, and in 2008, serving the care and education needs of heterosexually men of color, the Men’s HIV Prevention was added.Circle of Care is the HIV program of the Family Planning Council, and served over 2000 families annually. The Board of the Circle is comprised of consumers and representatives from the provider network, providing strategic direction and policy oversight. Circle of Care is the Ryan White C.A.R.E. Act, Part D grantee for the Philadelphia and surrounding area, and serves as a National model of family–centered care and services. Who We Are Circle of Care is the primary publicly funded HIV care system for families in Philadelphia. Founded in 1989, by providers and consumers, Circle of Care is built on a vision of integrated and consumer-centered services that are responsive to the priorities of individuals and families affected by HIV. Structured as a network of 16 clinics and support service providers, we currently serve over 1,900 individuals from 917 families. By bringing together the necessary collaborating organizations and agencies, the Circle of Care bridges the gaps in service systems for families affected by HIV. Circle of Care is the Ryan White Part D grantee for the Philadelphia area. The Part D Program focuses on the development and operation of systems of primary health care and social services that benefit children, youth, women and families living with HIV. Who We Serve Circle of Care’s target populations are: HIV positive women who have infected children; HIV positive women who have uninfected children; HIV positive and at-risk adolescents, and families with affected and infected infants and children. Circle of Care also has responded to the need to target and serve specific sub-populations of heterosexual men of color, women who are released from incarceration, women who are served in shelter systems, young MSM, and pregnant youth. |
